Rules and conditions governing use of the DIA website and related informational services

These Terms of Service (“Terms”) govern access to and use of https://www.health-life-advisors.com/ and related online content, forms, appointment tools, educational materials, recruitment information, and communications made available by Dedicated Insurance Advisors LLC. By accessing or using the website, you acknowledge these Terms. If you do not agree, do not use the website.

1. About DIA and the Website

DIA is a private insurance agency and insurance distribution organization. The website provides general educational information, access to licensed insurance assistance, appointment and contact tools, agent and agency partnership information, training information, and links to third-party resources.

No government affiliation  DIA is not affiliated with or endorsed by the United States government, CMS, Medicare, Medicaid, SSA, HealthCare.gov, or any federal or state agency. References and links to government programs are provided for education and convenient access to official resources.

2. Eligibility to Use the Website

You must be at least 18 years old to submit a general request, consent to marketing communications, request an insurance transaction, or apply for an agent or agency relationship through the website. A parent, guardian, household application filer, legal representative, or other authorized person may act for another individual only when legally authorized.

3. Informational Nature; No Professional Advice

Website content is general information and is not a substitute for individualized insurance, legal, tax, investment, accounting, medical, or government-benefit advice. Product illustrations, examples, educational articles, calculators, videos, and third-party materials may rely on assumptions and may not apply to your circumstances.

4. No Enrollment, Coverage, or Eligibility Created by Website Use

Submitting a form, scheduling a consultation, sending a message, downloading material, or speaking with DIA does not by itself:

  • Enroll you in or disenroll you from an insurance plan or government program.
  • Bind coverage, issue a policy, guarantee acceptance, or change existing coverage.
  • Confirm Medicare, Medicaid, CHIP, Marketplace subsidy, Social Security, or other government-program eligibility.
  • Create a Medicare Scope of Appointment, Marketplace enrollment authorization, carrier application, or permission to access government records.
  • Guarantee a premium, benefit, provider network, drug formulary, underwriting result, rate, commission, appointment, employment relationship, or business outcome.

Coverage becomes effective only according to the written terms, approvals, effective dates, and requirements of the issuing carrier, plan sponsor, Marketplace, or government agency.

5. Medicare-Related Services

Medicare Advantage, Medicare Part D, and other Medicare-related activities are subject to CMS rules, plan requirements, election periods, licensing, appointment, permission-to-contact, Scope of Appointment, documentation, and marketing restrictions. DIA and its agents will obtain additional documentation when required before conducting a regulated sales or enrollment activity.

DIA does not offer every plan available in every area. Organizations, products, benefits, service areas, provider networks, formularies, premiums, and availability may vary by location and contract year. 6. Medicaid, CHIP, Marketplace, and Social Security

Medicaid and CHIP are administered by state agencies under federal requirements. Applications, eligibility determinations, renewals, case status, benefits, and appeals must be handled through the applicable state agency or authorized government channel. DIA may provide general information or insurance assistance but cannot override or guarantee a government determination.

Marketplace assistance requires consumer consent and, when applicable, review and confirmation of application information. DIA will not knowingly enroll or change a consumer’s Marketplace coverage without the consumer’s legally sufficient authorization.

DIA does not represent SSA, administer Social Security benefits, promise a benefit increase, request payment for Social Security services, or control a my Social Security account. Use SSA.gov or other official government channels for Social Security matters.

7. Accuracy of Information You Provide

You agree to provide information that is accurate, current, and complete and to review information before an application or enrollment is submitted. You are responsible for promptly reporting changes that may affect eligibility, coverage, premiums, subsidies, underwriting, licensing, contracting, or communications. Do not impersonate another person or submit information without lawful authority.

8. Communications Consent and Preferences

When you request contact, we may respond by telephone, email, or text using the information you provide. Marketing consent, when requested, is separate from any insurance purchase or enrollment requirement. Message frequency may vary and message and data rates may apply. Reply STOP to opt out of text messages and HELP for assistance, or contact DIA directly.

Calls using a prerecorded, artificial, or AI-generated voice, and automated telemarketing texts or calls, will be made only when permitted by applicable law and after any legally required consent. You may revoke permission through any reasonable method. Revocation does not affect communications already lawfully made or necessary non-marketing service communications.

9. Agent and Agency Partnership Requests

Submitting a partnership or recruitment form does not create employment, an independent-contractor agreement, a carrier appointment, a commission entitlement, an exclusive relationship, or a promise of contracting. Unless a separate signed agreement expressly states otherwise, agent opportunities are independent, commission-based opportunities and not salaried employment.

Carrier contracts, commission levels, vesting, release rules, chargebacks, hierarchy, product access, lead programs, marketing support, and training obligations are governed by separate written agreements, carrier rules, and applicable law. DIA may verify licenses, appointments, background information, and eligibility before approving a relationship.

10. Annuities, Life Insurance, and Financial Information

Annuities and life insurance are insurance products, not bank deposits, and may involve surrender charges, limitations, exclusions, market-related features, tax consequences, and suitability or best-interest requirements. Illustrations and examples are not guarantees except as expressly stated in the issuing carrier’s contract. Product recommendations require individualized review and appropriate carrier documentation.

11. Third-Party Links and Services

Links to government agencies, carriers, providers, directories, enrollment systems, training platforms, social networks, and other third parties are provided for convenience. DIA does not control and does not warrant third-party content, availability, security, accessibility, privacy practices, or transactions. A link does not imply government affiliation, carrier endorsement, or DIA endorsement unless expressly stated.

12. Intellectual Property

The website’s original text, branding, logos, graphics, arrangement, training materials, and other content are owned by or licensed to DIA and are protected by applicable intellectual-property laws. You may view and print reasonable portions for personal, noncommercial use. You may not reproduce, republish, sell, scrape, alter, remove notices from, or commercially exploit protected content without written permission.

13. Prohibited Uses

You may not use the website to:

  • Violate law, regulation, carrier requirements, CMS or Marketplace rules, licensing obligations, privacy rights, or intellectual-property rights.
  • Submit false, fraudulent, misleading, unauthorized, harmful, or unlawful information.
  • Attempt unauthorized access, interfere with website operation, introduce malicious code, harvest information, or bypass security controls.
  • Impersonate DIA, an agent, a carrier, Medicare, Medicaid, SSA, CMS, or another government body.
  • Use website content to mislead a consumer or conduct unapproved insurance marketing or enrollment activity.

14. Website Availability and Changes

We may update, correct, suspend, restrict, or discontinue website content or functionality at any time. We strive for accuracy but do not guarantee that all content is complete, current, uninterrupted, or error-free. Official carrier contracts, plan documents, Evidence of Coverage, policy forms, government records, and applicable law control over inconsistent website content.

15. Disclaimer of Warranties

To the fullest extent permitted by law, the website and its content are provided on an “as is” and “as available” basis. DIA disclaims warranties not expressly stated in a written agreement, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. Nothing in these Terms excludes a warranty or consumer right that cannot legally be excluded.

16. Limitation of Liability

To the fullest extent permitted by law, DIA and its owners, officers, employees, and authorized representatives will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising solely from use of or inability to use the website or reliance on general website content. This limitation does not apply where prohibited by law and does not limit obligations expressly established by an insurance contract, applicable regulation, or separate written agreement.

18. Governing Law

These Terms are governed by applicable federal law and the laws of the State of Texas, without regard to conflict-of-law principles, except where another jurisdiction’s law must apply. Any dispute relating solely to website use may be brought in a court of competent jurisdiction in Harris County, Texas, unless applicable law requires another venue.

19. Changes to These Terms

We may revise these Terms when law, products, services, technology, or business practices change. Revised Terms will display a new effective date. Continued use after publication of revised Terms constitutes acceptance to the extent permitted by law.
